Alton Memorial Hospital:
Bar-coding is part of the Hospital's new
Medication Administration Check (MAK) system
that ensures the right patient receives the
right medication in the right dose by the
right route and at the right time.
UC San Diego Medical Center:
Hospital patients now get a
bar-code on their hospital
identification wristband which
improves how medication is
delivered to each individual
patient. The new barcoding
program, launched recently at
UCSD’s Thornton and Hillcrest
Hospitals, is the only such
system in the San Diego region,
outside of the Veterans Affairs
San Diego Medical Center.

University of Iowa Hospitals and Clinics: Hospital-wide
deployment of bar code technology to virtually eliminate the chance for
error in patient blood transfusions.
Brigham and Women's Hospital: Researchers at Brigham and
Women's Hospital (BWH) and Harvard Medical School, evaluating the use of
bar coding technology for storing and dispensing medication from the
hospital pharmacy, found that the rates of medication dispensing errors
and potential adverse drug events, which are dispensing errors that can
harm patients, were substantially reduced when bar code scanning
technology was used in the dispensing process. The findings appear in
the September 19, 2006 issue of the Annals of Internal Medicine.
Community Medical Center in Scranton, PA: Mobile technology is
an important aspect of CMC’s medication management process. Nurses
utilize a cart with a hand scanner (or a wand) and software that is
connected to the pharmacy. The computers on wheels (COWs) are rolled
between patient rooms by nurses who use the wand to scan each patient’s
ID bracelet and medicine. If the two do not match, an alert appears.

Massachusetts General Hospital: is among a growing number of
hospitals and laboratories implementing bar-code solutions to help
improve lab workflow, reduce lab errors, and improve patient safety.
Just as packages in FedEx warehouses are consistently scanned throughout
the delivery process to upload information into the company's
package-tracking system, at several points along the way in the MGH
pathology lab, bar codes on specimen slides are scanned, uploading data
into the laboratory's information system (LIS) and providing the lab
with a complete workflow history, including time-stamping and
accountability.

Memorial Hospital has seen a continued reduction in medication errors of
over 80% due to the implementation of bar coding technology for
medication administration. We have been using bar coding for medication
administration since August of 2003.
